Permission Forms

Please refer to the forms sent home with your child regarding different permissions. You will find the field trip consent form, the video/photography consent form, and the food consumption form. Please complete the video/photography consent form now. The field trip consent form should only be filled out if a field trip is planned. The food consumption form will be sent home with your child before any classroom party and must be completed prior to your child consuming party foods.
